Founded in 1995, Pharos Innovations, LLC (formerly Disease Management Systems, LLC) develops and markets proven solutions for patient monitoring.  Pharos Innovations has developed and clinically validated effective Internet and telecommunications services to assist payers and providers in managing their populations with chronic diseases.  Today, our products include patient monitoring for Congestive Heart Failure, Asthma/COPD, Diabetes High-Risk Pregnancy, Depression and Anticoagulation.  These products have proven effective in helping chronically ill patients modify their behavior to positively improve clinical outcomes and reduce overall healthcare utilization.

Through enrollment in the Tel-Assurance® program, patients can learn about, be screened for, and better track their disease on a daily basis. This is done with the use of Internet technology or an IVR telecommunications platform. By delivering daily information back to the provider, regardless of the platform, physicians and care providers act proactively to intervene when their patient's chronic condition begins to deteriorate. Pharos Innovations has become a leader in providing the connectivity to support this dynamic relationship between patient and provider.
