The US Government Has the Most to Gain from Disease Management Support

Chronic conditions are a leading cause of illness, disability, and death among Medicare beneficiaries and account for a disproportionate share of health care expenditures. About 14 percent of Medicare beneficiaries have heart failure but they account for 43 percent of Medicare spending.

According to the Centers for Medicare and Medicaid Services (CMS) estimates, patients with five or more chronic conditions compose 23% of the beneficiary pool but account for approximately 68% of spending.

CMS is currently evaluating ways to provide disease management which will eventually be a set reimbursable codes under Medicare.  Their evaluation process takes the form of various "demonstration projects."  Pharos Innovations is active in a number of these projects with compelling results.
