Background Data on Chronic Disease Heart Failure is the number one claim code by volume for hospitals, with an average admission length of over 5 days. Readmission rates for heart failure patients average 23% within 30 days of original discharge. Most hospitals LOOSE money on heart failure admissions. Chronic conditions are a leading cause of illness, disability, and death among Medicare beneficiaries and account for a disproportionate share of health care expenditures. About 14 percent of Medicare beneficiaries have heart failure but they account for 43 percent of Medicare spending. According to CMS estimates, patients with five or more chronic conditions compose 23% of the beneficiary pool but account for approximately 68% of spending. Chronic Care vs. Acute Care The healthcare system in the US pays providers to care for a heart attack patient. The same system also pays well for a diabetes related amputation. This model of acute care does not incent healthcare providers for prevention of such events or the management of chronic diseases that cause subsequent complications. Pharos Innovations was founded on the basis that the communication breakdown in the chronic care setting was the primary driver of poor health and increased costs in healthcare.
